Additional Information
- $190 for 50 minute and $95 for 25 minute session. Fees due at the time of services are rendered. Payment Options: Check made to Liza Chapman LMFT, cash, or Venmo. Payment collected at the beginning of session. Any additional services per week (collaborating with outside providers, phone consultation) will prorated per hourly rate.
- 48 hour cancellation policy on all scheduled appointments.
- All patients under the age of 13 must have a parent or guardian (caregiver) on site in the waiting area during the scheduled appointment for in person appointments
- Please no eating food, candy or gum during appointment time.
- Texting to set up / confirm appointments only. Any updates or personal information leave on voicemail.
